Access Coordination Services

 

Offered to Brooklyn residents at no cost.
We provide the following support:

 

  • Assist with the initial OPWDD, (Office for People with Developmental Disabilities) Eligibility Determination process;
  • Connect eligible individuals and their families to Non-Medicaid Service Coordinators, Care Coordinators and Family Support Brokers;
  • Support families with navigating the HCBS (Home and Community Based) Medicaid Waiver process;
  • Provide supports during assessments, life plan meetings and circle of support meetings;
  • Educate families regarding their choices and rights for OPWDD and school-based services;
  • Assist and troubleshoot navigating the self-direction program and OPWDD process;
  • Make referrals to clinical evaluations and state, city and private programs;
  • Link families to community resources;
  • Promote self-advocacy skills;
  • Provide educational group presentations and much more.
